a. How many grams of fat can you consume in a day and not exceed 30 percent of your calories from fat? Use the CNPP recommendation for your daily calorie recommendation to calculate your answer. How did you do in this area for the day you recorded?On the â€Å"standard† daily calorie intake of 2,200 recommended for very active young women, the upper limit for fat intake is 73 grams (660 calories). If I was to be sedentary man or an active woman, I would roughly have to meet the same requirement, which is not to take in more than 73 grams of fat in a dayb. How did you do in this area for the day you recorded?  I need to consume only ten percent of calories from saturated fat, meaning, I may not exceed more than 300 mg of saturated fats in day. Saturated fats such as cholesterol, and trans fat which is not only bad for my diet but will also clog up my aorta(major heart vein).c. For the day you recorded your intake if you ate a serving of a high-fat food, for example, lasagna, how could you avoid exceeding the recommended fat intake for the day?  During the event that I eat food containing high amounts of fat, to be able to balance the amount of fat intake, I eat food that contain less fat and eat foods that were boiled, broiled or grilled. In doing so, I end up taking in less fat and still eating a lot. Because even if we avoid frying food it will still most likely contain fat, so I will at least minimize the amount of fat included in everything else I eat.d. How could you change those ingredients to reflect a reduction in fat content?  I can reduce the fat content of lasagna by placing whole-wheat lasagna instead of the regular pasta, decreasing the amount of virgin olive oil that is placed in it, placing less butter, and also using ground chicken rather than ground beef.g. How did the day’s recorded total for calories and vitamins compare with your recommended amounts? Did the day’s meals meet or exceed your need for energy? Describe how your actual intake varied from the CNPP recommendation.  Happily, I was able to exceed the needed amounts for calories and vitamins for the day. As for the vitamins, I was happy to see that I exceeded it because it means that I am eating a healthy diet. Although for the calorie intake, it scares me a bit because eating a calorie pack food without expending it properly will result to added body fats.h. Did your meals present too little of any of the vitamins and minerals listed in the CNPP materials? Which ones?  Actually, my vitamin intake for the day exceeded what was expected from me greatly. I was able to take in more than what was required from me according to CNPP, which means none of them where under-represented.i. What changes in your choices among those foods would have improved the energy o r vitamin or mineral totals for the day?  Improvement when taken in a standard meaning would actually mean to increase, but in my case, to improve my diet I would have to decrease my energy intake and simply maintain my vitamin and mineral intake for the day.j. Did your choices provide enough folate to meet your requirement?  I am glad to say that I actually met the required amount of folate that I had to take in within one day. In fact I was able to take in twice the required amount of 400 micrograms and was able to take in 784.5 micrograms.k. What are the sources of niacin in your day’s meals?  Niacin is not really that abundant in standard food. Niacin can actually be found in whole refined grains. When mills refine grain they are required by law to add folic acid, NIACIN, iron and riboflavin. So therefore eating refined grains at least once a day may add to my Niacin intake.l. What about Vitamin C? What percentage of your daily need of Vitamin C did your meals provi de? Which individual foods were the main contributors? To what food groups do they belong?  I was able to take in a total of 126.78% vitamin C within one day. Vitamin C or Ascorbic Acid is abundant in Citrus filled drinks and meals such as lemons, oranges and lime, all of which are in the Fruits part of the Food Pyramid. Also aside from normal food intake, it is easy to receive vitamin C by simply buying it in a local drugstore.m. How did your total energy intake compare with your energy recommendation? Is this consistent with your nutritional goals?  In my food intake, I was able to take 6604 kcal worth energy, which is three times more the recommended 2339 kcal goal for a day. It is very impressive to see that I was able to take in that much energy. However, it is also true that if I am not able to expend that energy within a day, it turns to body fat which is terrible to have.n. Which of your foods are â€Å"vitamin bargains†? Those would be foods which are vitamin-den se, providing the most vitamins for the fewest calories.  Fruits and vegetables of course give me the most amounts of vitamins without having to increase my total number of calories significantly. In fact it can be noted that the best source of vitamins are vegetables because they don’t contain fructose like fruits do, but they contain a minimal amount of starch which makes up a good low calorie diet.o. Breakfast cereals are a great source of vitamins. What characteristic of these foods makes them so rich in vitamins?  Breakfast cereals are made from plants specifically corn, wheat, and grains. Corn in itself is full of vitamins and wheat and grains may contain some vitamins as well. In addition, the fact that when we eat cereals it always comes with full cream or filled milk, which adds a significant amount of vitamins in a persons diet.p. What can you say about your recorded food intake and the vitamins and minerals that you obtained from the foods you ate that day?   I can say that I actually exceed the requirements that are set for a balanced diet. One of these issues deals with using marijuana as medicine to treat patients. There has been controversy over medical marijuana ever since it was discovered that patients respond to it. There are many arguments supporting the use of medical marijuana in the health care field and many arguments in opposition of its use. The United States Congress classifies marijuana as a Schedule I substance meaning it has high abuse potential and is of no medical value. However, â€Å"physicians and the general public alike are in broad agreement that marijuana shows promise in combating diverse medical illnesses â€Å"(Bostwick, 2012). As a nurse, it is important to understand both the pros and cons of medical marijuana before forming an opinion on the issue. Having all the information necessary will allow a nurse to better care for a patient in circumstances where their beliefs and values are different from your own. The number one benefit of marijuana in healthcare is its use in treating symptoms that cannot be controlled as well by other medications. One example of this that has been supported by extensive research is in patients who are undergoing chemotherapy for cancer who have reported that marijuana controls symptoms of nausea and vomiting, increases appetite, and improves sleep. One study, in which 362 participants undergoing chemotherapy who had breast, colon, lung, lymphoma, or other malignancies showed that â€Å"cannabinoids are more effective than placebo and comparable to antiemetics such as prochlorperazine and ondansetron for chemotherapy induced nausea and vomiting† (Cotter, 2009). Marijuana is also useful for a variety of psychiatric disorders. According to an article from Clinical Psychiatry News, â€Å"about 75% of respondents said they used marijuana for psychiatric disorders, including bipolar disorder, posttraumatic stress disorder, depression, anxiety, and persistent insomnia. Unlike some psychiatric drugs, they said, marijuana didn’t leave them feeling like zombies† (Otto, 2012). Another way marijuana is medically beneficial is in stimulating appetite in patients experiencing weight loss and wasting syndrome associated with AIDS. The results of one experiment, in which THC in the form of dronabinol was studied in patients with AIDS-associated cachexia, â€Å"showed it effective in increasing weight as well as being well tolerated† (Hollister, 2001). These are only a few examples of how medical marijuana has been of benefit. There are numerous other conditions and diseases in which marijuana has been proven to be effective including Alzheimer’s disease, epilepsy, Multiple Sclerosis, Glaucoma, Arthritis, Depression, Anxiety, and Hepatitis C. While marijuana is clearly beneficial in many ways, it is also important to consider the negative effects. One concern is the negative effects of smoking marijuana on the lungs. According to an article from Canadian Nurse, â€Å"Marijuana smoke contains more tar and toxins than tobacco smoke. One joint is equal to two and a half cigarettes in terms of lung damage† (Halpern, 2010). Smoking marijuana over a long period of time can lead to bronchitis and emphysema. Although there are oral â€Å"cannabinoid† medications to prevent problems to the lungs, the oral forms can also have negative side effects including dizziness, sedation, and intoxication. Another negative component to consider is the possibility of becoming addicted to marijuana, which is a big concern to those who use marijuana frequently for therapeutic and nontherapeutic uses. The ethical issue of medical marijuana can have an impact on a nurse’s ability to perform their job. For example, if a patient comes in and lets a nurse know they are smoking marijuana for symptom control because it is the only thing that helps and yet a nurse knows they are acquiring the drug illegally, it poses questions as to whether or not the nurse should support the patient’s continued use or try to talk them into alternatives. A nurse should also be concerned as to whether it is breaking the law and if there could be legal ramifications for not turning the patient in. Many agencies deal with these concerns by offering nurse’s the option to refuse to assist a patient if a situation arises that violates their religious or moral beliefs. However, this is only true in cases where a patient is in no danger. While the issue of marijuana use is still controversial today, a total of eighteen states in the US as well as Washington, DC have enacted laws to legalize medical marijuana. Daniel Golemans approach consists of six major styles of leadership. In his book Primal Leadership, he has highlighted that good leaders are effective because they create resonance. Based on this he explained resonance can be done in six ways, which in turn lead to the leadership styles. These styles included visionary leadership, coaching leadership, affiliative leadership, democratic leadership, pacesetting leadership and commanding leadership (Goleman, 2006). Thus Goleman argues that true leaders are a product of the decisions that they take and thus a true leader is one who thinks through all the factors before taking a decision and after considering every possibility. Another aspect that needs to be considered in any organization includes the training of employees. Training a group involves a lot more efforts as well as costs than training a single employee. However when training an individual, it is possible to help them out on improving the process and finding ways to overcome issues. This cannot be done in the group training (House Shamir, 1993) . Training an individual can be done while working on the job which allows the employees to also get a feel of the systems and ask questions which they would have otherwise been reluctant to ask in a group. The half truth that has been used by managers in a number of ways is that people are human resources (Locke, Edwin, 1975). Mintzberg argues and highlights th at it is incorrect to refer to human beings as human resources. He argues that half truths are dangerous mainly because of the fact that they can affect the actions of the leaders to be not well thought out and planned (Grint, 2001). Also the half truths are not reasonable and require to have been taken as a huge threat to the managers and leaders of organizations. Of all the different leadership modes that are present in various organisations, one which has proved to be very effective and efficient is that of a charismatic leader. In a situation where decisions need to be taken very fast and with accuracy, the most effective leader would be the charismatic leader (House Shamir, 1993). Charismatic leaders mainly refer to people with an elusive and also an indefinable personality trait which in a number of terms seems unnatural and is considered to be supernatural. These traits have been expressed to be traits like ability to lead, charm, persuade, influence and inspire others (Beer, Lawrence, Quinn Mills and Walton, 1985). According to Weber: charisma is ‘a certain quality of an individual personality, by virtue of which s/he is set apart from ordinary people and treated as endowed with supernatural, superhuman , or at least specifically exceptional powers or qualities. These are such as are not accessible to the ordinary person, but are regarded as of divine origin or as exemplary, and on the basis of them the individual concerned is treated as a leader’. He also says, resting on devotion to the exceptional sanctity, heroism or exemplary character of an individual person, and of the normative patterns or order revealed or ordained by him. Every leader irrespective of whether a charismatic, authoritative or even a transformational requires to have a team and followers to be able to lead them. Hence it is clear that a leader only leads the way and helps the others to follow him and meet the goals of the organization. In situations where the leaders require having complete support of the followers, the most effective form of leadership again is the charismatic leadership (House Shamir 1993). The transformational leaders focus on transforming the organisation and in some cases the followers as well, while the charismatic leaders prefer to let things remain the same and do not want to change things. In the case of charismatic leaders it is quite easy for the followers to get carried away while talking to the person because of the strong aura that they have. The charismatic leaders are a combination of both people as well as organisation oriented. Hence the charismatic leaders to a great extent are great leaders and provide a lot of results to the organisation as a whole. Conclusions: As seen from the above discussion, the leaders play a major role in the over organizational behavior and the overall effectiveness of the organizations. The ability to understand the needs of each individual and to work towards providing all employees with the right treatment to be motivated is the main job and role of the leader. Extraction of Impacted Mandibular Third Molars INTRODUCTION The extraction of impacted mandibular third molars is a common  procedure in oral and maxillofacial surgery. The reasons for extracting  these teeth include acute or chronic pericoronitis, presence of cysts or a  tumour, periodontal problems and presence of a carious lesion on the  second or third mandibular molar. In some cases, extraction is performed  in preparation for orthodontic treatment or orthognathic surgery  In most cases, the removal of third molars will lead to a significant degree  of tissue trauma that causes an inflammatory reaction. Thus, the patient  develops the common postoperative symptoms and signs of pain, facial  swelling, dysfunction, and limited mouth opening (trismus). The pain is typically brief and will peak in intensity in the  early postoperative period.The facial swelling and trismus will reach their  characteristic maximum 48 to 72 hours after surgery. Those symptoms are  major disadvantage and affect the patients quality of life. The  inflammatory process is necessary if healing of traumatic tissue has to  occur, but often excessive inflammation lead to unnecessary pain, trismus  and swelling. The impact of these symptoms affect the quality of life in the  days following surgery. Injured tissues immediately release local inflammatory mediators, like histamine, that produce vasodilatation leading to extravasations resulting in edema and sensitize the peripheral noci-receptors resulting in hyperalgesia. Although these inflammatory mediators are released immediately after the trauma, these symptoms are not observed immediate after the surgery but rather begin gradually, peaking 1 3 days after the surgery. Pain has been an indispensible part of all surgical procedures and minor oral surgical procedures are not an exclusion. Through ages mankind has been in constant quest for various methods to control pain . The specificity theory proposed in 17th century by Descartes R explains pain as the activity of highly specific peripheral nerve endings that receive sensory information from the environment, which is then transmitted by nerve fibres through the spinal cord to the pain centre, or the pineal body, in the forebrain. However it is merely a biological explanation and does not address the multidimensional, complex process of pain as we understand today.1 Various factors contribute to determine the intensity of post-operative complications such as host defense mechanism, type of healing, duration of the procedure, 4-8extent of reflection of the mucoperiosteal flap, types of flaps, bone removal, need for tooth sectioning, 5and experience of the surgeon.9,10 To increase patients satisfaction after third molar surgery it will be necessary to avoid the inconvenience associated with tooth extraction and minimize the subsequent side effects. Methods to reduce the side effects is to prescribe medications such as corticosteriods , non-steroidal anti-inflammatory drugs , a combination of corticosteroids and non-steroidal anti- inflammatory drugs or enzyme preparation. Synthetic inflammation inhibitors which are active at certain points during the course of the inflammatory reactions are available. They are sometimes associated with undesirable side effects such as insomnia, depression, systemic fungal infection, increased calcium excretion, gastrointestinal irritation, visual complaints, fever and fatigue. These inflammation inhibitors are divided mainly into two groups: Steroids, i.e., cortisone and its derivatives, and the Non-Steroids, which are usually salicylic acid derivatives such as butazolidine, indomethacin and others. The side effects of prolonged corticosteroids use are well known, and are fundamentally dependent upon the dose employed and the duration of treatment. These effects include peptic ulcer, immune suppression, water and electrolyte balance metabolic effects, muscle atrophy, osteoporosis, increased fatty tissue (full moon facial appearance), Cushing syndrome, avascular osteonecrosis, lessened resistance to infection, hirsutism, amenorrhea, acne, hyperglycemia or hypertension. Systemic glucocorticoids , which are frequently used as anti-inflammatory agents, are well-known to inhibit wound repair via global anti-inflammatory effects and suppression of cellular wound responses, including fibroblast proliferation and collagen synthesis. Systemic steroids cause wounds to heal with incomplete granulation tissue and reduced wound contraction. Glucocorticoids also inhibit production of hypoxia-inducible factor-1 (HIF-1), a key transcriptional factor in healing wounds Non-steroidal anti-inflammatory drugs (NSAID) have been used since many years for their analgesic and anti-inflammatory properties. Although these drugs have been proven efficient in management of post operative pain, adverse effects and associated morbidity pose a serious problem. It has long been known that NSAID may have a range of side effects, of which the commonest are gastrointestinal.5 Non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen are widely used for the treatment of inflammation and rheumatoid arthritis and for pain management. Low-dosage aspirin, due to its anti-platelet function, is commonly used as a preventive therapeutic for cardiovascular disease, but not as an anti-inflammatory drug. In contrast to chemical inhibitors of inflammation such as nonsteroidal anti-inflammatory drugs (NSAIDs), enzyme preparations support and accelerate the natural inflammatory process without contributing to pain, redness and swelling. This is accomplished by helping degrade and remove plasma proteins and fibrin that invade the interstitial space within tissues at the site of inflammation. Improved microcirculation and removal of inflammatory products results in an analgesic effect and complaint relief. Proteolytic systemic enzymes (proteases), such as those found in bromelain, papain and pancreatin, cleave protein compounds by hydrolysis. That is, they split their substrate by incorporating water.
